· LICENSE # (10 Digit)
· NRDS# (9 digit)
· Local Board Association – Currently ACTIVE (or copy of application)
· Valid EMAIL ADDRESS (will not add agent without a valid email on file)
REMOVING AGENT– COMPLETE LOWER PORTION OF THE FORM (include Broker/Manager Signature)
· Name as Appears on LICENSE
· LICENSE # (10 Digit)
· Effective Date (for Inactive Status)
· MUST Indicate if License Sent Back to STATE (include date) or In HOLDING Co.
· MUST Indicate WHERE ACTIVE, PENDING, ETC. LISTINGS ARE ASSIGNED (i.e. Back to Listing Broker, Transfer to Another Agent in the Firm, or Remain with the Agent Leaving and going to his/her new office).
· Agents may also have CONTACTS, CMA’s, and/or SEARCHES that can either remain with the “agent file” or be moved to the Broker or other assigned agent – this area of the form MUST be completed to allow MLS Staff to process the change in agent status in a timely manner.
AGENTS WILL NOT BE ADDED OR REMOVED WITHOUT COMPLETION OF THE REQUIRED AGENT STATUS CHANGE FORM – INCLUDING THE AUTHORIZED SIGNATURE OF THE BROKER/OWNER OR MANAGER.
